What You’ll Need to Apply for a Scholarship

Whether you're new to our scholarships or renewing, we're here to support you—no matter where you're stationed. This guide will help you determine which documents to include with your child’s application to confirm Florida residency.

If your military family currently resides in Florida—whether you're applying for the first time or renewing—you’ll need to provide proof of residency using documents from the Proof of Florida Residency chart below.

Purchase pre-approved services and products for your child’s education and request reimbursement.

To meet residency requirements, submit:

One document from Column A

Two different documents from Column B

*Important Notice: Any completed and eligible FES-UA application submitted after December 15, 2023 that count towards the program’s statutory cap will be considered as pre-registering for the 2024-25 school year.

Proof of Florida Residency

Proof of Residency requires one document from Column A or two different documents from Column B. All documents must be current (dated within two months for recurring bills or statements), valid, and match the parent's or guardian's (primary or secondary) full name and the physical address on the application. 

Column A
Upload a copy of one of these documents

Utility bill (electric, gas, water)
Mortgage statement or residential lease agreement
Proof of current government benefits (Social Security, Veterans Affairs, Disability, Medicare, Section 8/HUD, TANF, SNAP, DCF correspondence)
Homestead or Property tax statement/assement
Verification of homelessness
Migrant Address Verification Letter

Column B
Upload a copy of two of these documents

Florida Driver's License or state-issued ID card
Paystub
Insurance policy statement (auto, health, home, or renters)
Mortgage acceptance letter
Property Deed
Please note: Both documents from Column B need to be from the same guardian and must be from different categories.

All documents must:

  • Be current (dated within the past two months for bills or statements)
  • Clearly show the full name of the parent or guardian (primary or secondary)
  • Match the physical address listed on your scholarship application

When you indicate that your child is a dependent of an active-duty armed forces member, you will be asked to submit documentation. You can use Permanent Change of Station orders, a Leave and Earnings Statement, or an IRS DD Form 2058.

New to the Program & Stationed Outside Florida?

If your family is applying for the first time and has a Permanent Change of Station (PCS) order to Florida, simply upload a copy of your PCS as the Proof of Residency in your application.

Relocating Out of Florida?

This only applies to renewing families funded from the Unique Abilities or PEP Scholarship during this school year.

Already receiving a scholarship but preparing to move out of state? You still have options to continue receiving funds for your child’s education.

To confirm your proof of residency, please list the address of the Florida base from which you were relieved in your EMA profile. Please provide your Permanent Change of Station orders as your proof of residency document.

To confirm that your home of record or legal residence remains Florida, be sure to provide either:

  • Your Leave and Earnings Statement (LES)
    or
  • IRS Form 2058 (Domicile Declaration)
